How linking social services with medical care improves Oregon’s health equity


Sometimes better health comes down to having the right kitchen equipment.

That was true for one Mosaic Medical patient recently. When the patient’s dietitian learned the individual couldn’t roast vegetables because they didn’t have a baking pan, the dietitian tapped into the coordinated care network called Connect Oregon to look for a solution.
